Submission Checklist for Bidders

When submitting a bid, ensure the following "basic paperwork" is included to avoid being ruled ineligible:

Signed W-9 Form for the current year.
Certificates of Insurance (Worker’s Compensation and Limited Liability).
IRS Tax Compliance Report (Letter 6201 or 6574).
Indiana Business Entity Report.
Local Permits/Licenses for the specific County or City.
